Bone Fit™ Workshop Informed Consent Form RELEASE AND ACKNOWLEDGEMENT – PLEASE READ CAREFULLY
I, , have registered in a Bone Fit™ Workshop offered through Osteoporosis Canada & Ontario Osteoporosis Strategy (“the Workshop”). In consideration of being able to participate in the Workshop, I acknowledge and agree to the following.
Assumption of Risks and Waiver of Liability
I am over the age of majority, in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in the Workshop. My enrollment and subsequent participation in the Workshop is purely voluntary.
The Workshop may involve strenuous physical activity and participation in this Workshop many involve risks and dangers which are inherent to fitness activities. I understand that I may injure myself as a result of my enrollment and subsequent participation in this Workshop. I hereby freely and fully assume all such risks and dangers associated with the Workshop.
I waive any and all claims that I have or may in the future have against Osteoporosis Canada & Ontario Osteoporosis Strategy (collectively “Osteoporosis”), the Workshop lead trainer, co-trainers and any of their agents or representatives (collectively the “Released Parties”) from any liability whatsoever arising out of or in connection with any activity within the scope of the Workshop, including any liability for any loss, damage, injury or expense due to negligence, breach of any other duty of care, or failure on the part of Osteoporosis Canada to take reasonable steps to safeguard or protect me from the risks of participating in the Workshop.
I will not make any claim or take any proceeding in connection with any of the claims released by virtue of this form, against any other person, firm or corporation which might claim contribution or indemnity from Osteoporosis Canada by virtue of such claim or proceeding. If I am sued as a result of my participation in the Workshop, I understand that signing this form precludes me from seeking contribution and indemnity from the Released Parties pursuant to any statute or at common law.
Acknowledgement of Copyright and Intellectual Property Rights
I recognize and agree that the Workshop curriculum associated materials and information created by Osteoporosis Canada or another entity for use and distribution in the Workshop (the “Workshop Materials”), and any and all copyright, trade mark and other intellectual rights in the Workshop Materials, remain the exclusive property of Osteoporosis Canada or the other entity. The Workshop Materials include, but are not limited to, the Bone Fit™ logo, the Osteoporosis Canada logo, the Ontario Osteoporosis Strategy logo, the Bone Fit™ Basics & Clinical Workshop binder materials and brochures, including all graphics, diagrams, photographs, and texts.
I understand that Osteoporosis Canada has not assigned or granted to me any interest in the Workshop Materials.
I will not sell, modify, edit, adapt, or otherwise make unauthorized use of the Workshop Materials. I understand that items that have been specifically identified as shareable by Osteoporosis Canada or the Workshop trainer can be photocopied “as is” but cannot be modified in any significant manner. I agree not to make unauthorized reproductions or misrepresent any portion of the Workshop Materials as my own. I acknowledge that I cannot use the Bone Fit™ logo in any of my promotional materials or on any form of media, including social media without express prior consent.
Social Media Promotion Limitations
I understand that I have the right to promote myself as Bone Fit™-trained only after successfully completing all of the requirements of the Bone Fit™ Workshop and passing the final quiz. I cannot promote myself or my business on any of the social media pages of Bone Fit™, Osteoporosis Canada, or associated entities. I acknowledge that Osteoporosis Canada, its employees, and associates are under no obligation to provide me with any marketing or promotional support at any time.
By my signature, I indicate that I have read and understand this Informed Consent Form. I understand that by signing this document I am waiving certain legal rights, including the right to sue or claim compensation following an accident, and I voluntarily agree to its terms. I hereby affirm that I have read and understand the above statements.

All personal information is subject to security measures. We will not share your information with any third parties. We do not divulge information to other organizations or people who might wish to access our membership for the purpose of self or product promotion under any circumstances.
We will honour any request you may have to access or review the personal information that you have given to Bone FitTM in the course of your membership.
Cancellation Policy:
If notified 2 weeks prior to the Workshop, an administrative fee of $50 will be charged and remaining course fees will be reimbursed.
If an individual cancels within 2 weeks of the workshop or does not attend the workshop, the course fees will be non-refundable.
